Referral Forms
Most Managed Care Plans require referral forms for specialists and/or diagnostic testing. We will complete your referral forms Monday through Friday, 8am to 4pm. In most cases, referral forms will be completed at the time of your office visit. If you have a scheduled appointment that requires a referral form, please give us 7 days advance notice to complete your referral form. If you have an elective surgery scheduled, please give us two weeks advanced notice to complete referral forms. You are responsible for obtaining your referral form prior to your scheduled appointment. Unless your managed care plan allows you to go directly to a specialist, if you access specialty care without a prior written referral, you will be responsible for the bill associated with the care. We are not able to issue retroactive referrals for care already rendered. All referrals can be picked up at our office. No faxing or mailing of referrals, except in extreme emergencies.
